Case Management Specialist | Healthcare Account (Hybrid)

Responsibilities: • Conduct initial assessments of patients' medical conditions, care needs, and treatment goals. Develop Healthcare plans tailored to each patient's requirements. • Administer medications, treatments, and medical procedures as prescribed by healthcare providers.
